Supervision of children in the [one-year-old classrom] group was inadequate in that [toddlers were able to bite others]. CCF Handbook, Section 2.4 General Requirements [SR] Licensing Specialist received a report of children biting in the one-year-old classroom. Licensing Specialist interviewed staff and determined a lack of supervision during diaper changes and playground time. Technical assistance provided from the CCP Handbook: 2.4 Supervision 2.4.1 General Supervision Requirements A. Child care personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised. B. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location. Corrective action: Provider had meeting with the one-year-old classroom to discuss recent issue with biting and new practices to illuminate future incidents. Director reviewed biting policy and staff signed and placed in employee file. Director will provide extra coverage in classrooms during diaper changes and playground time. Director will enforce expulsion policy for continuous incidents occurring in one day. Adding additional staff brought this standard into compliance. Due Date Completed at time of inspection

Supervision of children in the [Infant] group was inadequate in that [Staff was continuiously on cell phone for personal use and not in eyesight of infants in care]. CCF Handbook, Section 2.4 General Requirements [SR] Licensing Specialist observed staff constantly using their cell phones for personal use. Licensing Specialist observed staff having conversations with each other and not in sight of the infants in cribs, bouncy, and on the floor. Technical assistance provided from the CCF Handbook: 2.4 Supervision 2.4.1 General Supervision Requirements A. Child care personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised. B. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Corrective Action: Director is to have a staff meeting to discuss cell phone policy and supervision requirements concerning children. A method of discipline was used at the facility that was severe, humiliating or frightening to children in that [a staff member used extreme force to make a child stay on their assigned sleeping mat during naptime.]. CCF Handbook, Section 2.8, A and s.402.305 (12), F.S. General Requirements [SR] Licensing Specialist reviewed camera footage provided by Director. Licensing Specialist observed staff member lift the child with one arm from the ground and body slam the child on the sleeping mat several times during the observation. Licensing Specialist observed the staff member continuously applying pressure to the childs head with her hand with force and with harsh verbal tones. Licensing Specialist observed staff members completely covering the childs head with a blanket during naptime. Licensing Specialist observed the staff member placing her leg over the childs back to restrain the child from moving and getting off the sleeping mat Technical assistance provided from Florida Statutes: s402.305 (12), FS - Licensing standards; child care facilities. (12) CHILD DISCIPLINE. (a) Minimum standards for child discipline practices shall ensure that age-appropriate, constructive disciplinary practices are used for children in care. Discipline shall not be associated with food, rest, or toileting. 3. Spanking or any other form of physical punishment is prohibited. (b) Prior to admission of a child to a child care facility, the facility shall notify the parents in writing of the disciplinary practices used by the facility. The nap/sleep space was under furniture or against furniture that created a hazard. CCF Handbook, Section 3.6.2, D Physical Environment [SR] The infant room diaper changing table is attached to the wall and is lifted down when in use by staff. It is attached 4 feet from the ground. Licensing Specialist observed a childs sleeping mat underneath the diaper change table. Licensing Specialist observed staff members changing diapers while child was sleeping underneath. Technical assistance provided from the CCF Handbook: 3.6.2 Nap/Sleep Space D. Napping and sleeping spaces must not be under furniture or against furniture that creates a hazard. Corrective Action: Director requires all children in infant room to sleep in cribs until they are transitioned to the one-year-old classroom and are trained to sleep on a mat. Child/ren were placed in a crib, playpen, play yard or other sleeping and napping bedding with items that could pose a strangulation or suffocation risk. CCF Handbook, Section 3.6.4, A Physical Environment [SR] Licensing Specialist observed child covered completely with a blanket during naptime by staff members. Licensing Specialist observed the child struggling at times to remove the blanket from her head. Director advised Licensing Specialist that covering children completely with blankets is not their practice or policy. Licensing Specialist observed infant in crib with bottle. Technical assistance provided from the CCF Handbook: 3.6.4 Safe Sleep A. All personnel that care for infants must follow safe sleep practices as recommended by the American Academy of Pediatrics (AAP) as referenced in Caring for Our Children Basics Health and Safety Foundations for Early Care and Education, which is incorporated by reference in 65C-22.001(7)(v), F.A.C. Cribs or playpens/play yards used for infants must have tight fitted sheets and no excess bedding, which includes but is not limited to: bumper pads, hanging mobiles, quilts, comforters, receiving blankets, pillows, stuffed animals and cushions. Corrective Action: Director advised staff of safe sleep practices and no blanket allowed in infant room. Director will require infant staff to complete the safe sleep course online as a refresher. Items unrelated to diaper changing were stored in the diaper changing area or placed on the diaper changing table/surface. CCF Handbook, Section 3.10.2, G Sanitation and Equipment [SR] Licensing Specialist observed staff members placing their lunch on the diaper changing table. Licensing Specialist observed that the changing table surface was not cleaned after each diaper change. Licensing Specialist observed staff not using gloves during diaper changes or washing their hands after diaper changes. Technical assistance provided from the CCF Handbook: 3.10.2 Diapering Requirements A. When children in diapers are in care, a hand-washing station that includes a sink with running water, soap, trash receptacle, and disposable towels or hand drying machines that are properly installed and maintained shall be provided in the room or in an adjoining area which opens into the room. Hands must be washed and dried thoroughly after each diapering or toileting procedure, and all surfaces that have been touched must be cleaned and sanitized or disinfected to prevent the spread of germs. C. Handwashing sinks must not be used for food service preparation, dishwashing, or food clean up. D. The diaper changing area must be physically separated from the food preparation, food service, and feeding area. E. When children in diapers are in care, there must be a diaper changing area with an impermeable surface that is cleaned and sanitized or disinfected after each use. F. Staff was in the classroom passing out snack. Director sent child to classroom and advised staff to call for someone to take child to restroom. CCF Handbook, Section 6.4, A Health Requirements [SR] Child was given whole milk by accident that had a written restriction from his physician to not serve. Parent was not notified that this incident occurred. Staff did not complete an incident report to notify parent. All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur. A prescription or non-prescription medication, specifically [diaper powder], was dispensed without written authorization from the custodial parent or legal guardian. CCF Handbook, Section 6.5, A and K Health Requirements [SR] A staff member noticed red bumps on child's bottom. Staff member applied diaper powder to butt area. This powder was not authorized by parent to use. The daycare supplies the diaper powder. Licensing Specialist advised provider only soap and water can be used. Any other substance or medication requires a written authorization and must be provided by parent. Technical assistance provided from the CCF Handbook: 6.5 Medication Child care programs are not required to give medication; however, if a program chooses to do so, it must comply with the following requirements: A. The child care program must have written authorization from the custodial parent or legal guardian to give prescription and non- prescription medications. This authorization must be dated and signed by the custodial parent or legal guardian and contain the childs name; the name of the medication to be given; and date, time and amount of the correct dosage to be given. Prescription and non- prescription medications that are used on an as needed basis require the parent/ legal guardian to provide additional documentation on the authorization form to describe symptoms that would require the medication to be given.
