      Based on record review and interview at 12:27 p.m., S1 failed to ensure the centers children’s daily attendance records included the child’s arrival and departure times, and first and last name of person or entity to whom the child is released to each day. The Specialist requested to review the daily attendance records for 02/12/2026 and 03/03/2026. On 03/03/2026 and 02/12/2026, documentation showed that arrival times, departure times were not listed, and the first and last name of the person to whom the child is released to is not listed. Mandated Reporter: Based on record review/interview at 1:00 p.m., S1 failed to ensure all staff members completed the Mandated Reporter training annually. 1 of 10 staff members, S6 (DOH 09/06/2024 did not have current certification. S1 stated she thought S6 had completed the training, but found that she had not completed it. S1 stated each staff member will have proof of Mandated Reporter Training by 01/24/2025.

The documentation presented only showed that tornado drills had been done in the months of March, April, and May. In addition, documentation for the month of March failed to include signatures of staff present.

S1 failed to have documentation that the excursion was conducted at the center.
