Based on record review/interview at 9:45 a.m., S1 failed to have a log, either handwritten or in electronic form, or other written documentation of monitoring of provisionally-employed staff member, S8. Documentation did not include S8, the designated monitor for each, and the times of the visual observations from 3/01/2026 until present (3/25/2026). Corrective Action: Effective 3/25/2026, S1 stated she or another designated staff will document the monitoring of provisionally-employed staff to ensure compliance with this regulation.
