Based on observations/interview at 11:00 a.m., S1 failed to ensure the indoor area was free of hazards as evidence of the floor being cracked and separated in the hallway near Room 4 and the entrance/exit of the restroom utilized by children. Corrective Action: Effective 5/29/2025, S1 stated she will have the floors repaired or place an anti-slip rug/runner in the hallway to ensure compliance with this regulation.
