2.: Based on observation at 11:15 a.m., S1 failed to have the manufacturer's restraint device at an infant table. There were 4 infants at a table being fed that had no strap and buckle to secure them in the seat. Corrective Action: Effective 10/29/2025, S1 stated she will place an order by the end of the day for the restraints to ensure compliance with this regulation.

Based on record review and interview at 11:15 a.m., S2 failed to have documentation of tornado drills that were conducted at least once per month during the months of March, April, May, and June. S2 stated he was unsure where he placed the documentation of the tornado drills. Corrective Action: Effective 10/29/2025, S2 stated he will ensure to document and keep records of tornado drills to ensure compliance with this regulation.

Based on observations at 12:00 p.m., S1 failed to operate within the licensed capacity for the center. The center capacity is 42, and there were 46 children present. Corrective Action: Effective 07/23/2025, S1 will ensure to operate within the licensed capacity for the center to ensure compliance with this regulation.

Based on record review/interview at 12:30 p.m., S1 failed to maintain the center's daily attendance record for children as it did not accurately reflect the children on the child care premises at any given time. The Specialist observed 46 children present, and only 39 children were signed in on the log. Corrective Action: Effective 07/23/2025, S1 stated he will review the children's attendance records each morning to check for accuracy and ensure compliance with this regulation.

Based on record review/interview at 12:15 p.m., the center's staff and owner's daily attendance record failed to accurately reflect persons on the child care premises at any given time. S6 and S7 were signed in on the attendance records at 9:00 a.m., but were not present at the center upon the Specialist's arrival at 12:00 p.m. S6 and S7 also failed to sign back in when they arrived back at the center at 12:15 p.m. Corrective Action: Effective 07/23/2025, S1 stated he will meet with staff by the end of the day to review documentation of attendance to ensure compliance with this…

Based on observation/interview at 10:45 a.m., although the center has a behavior management policy, S5 (DOH 10/26/2023) used a prohibited method of discipline. On 06/23/2025, at 10:45 a.m., C1, three-year-old, was subjected to verbal abuse as the Specialist heard C1 crying and then heard S5 tell C1 to "shut it", in a harsh tone. Corrective Action: Effective 06/23/2025, S1 stated she will meet with S5 by the end of the day to review the behavior management policy to ensure compliance with this regulation.

Based on observation/interview at 10:15 a.m., S6 failed to have bottle formula/breast milk for infants labeled with the children's name. The Specialist observed 2 bottles in the infant classroom with no names or identification on the bottles. The bottles were labeled prior to the Specialist's departure. Corrective Action: Effective 06/23/2025, S1 stated she will meet with staff by the end of the day to review bottles being labeled to ensure compliance with this regulation.

Based on observation at 11:30 a.m., on 04/22/2025, S3 permitted a bib to be worn by a child while asleep. The Specialist observed one infant in a crib asleep with a bib on. S3 removed the bib immediately upon the Specialist's request, prior to the end of the visit. Corrective Action: Effective 04/22/2025, S1 stated she will review safe sleep with staff by the end of 04/22/2025, to ensure compliance with this regulation.

Daily Attendance Records Children Based on record review/interview at 12:00 p.m., S1 failed to ensure the centers daily attendance record accurately reflected the children on the child care premises at any given time. S1 stated that C1, one year old, was present and checked out on the morning of on 02/19/2025 for a doctors appointment and returned the next day.
