1501.A Based on record review and observation at 10:15 a.m., S1 failed to notify the Licensing Division prior to making changes that had an effect on the license as the center is licensed for a capacity of 37 children and 42 children were in attendance on 5/28/2026 without prior approval by the Licensing Division. Corrective Action:

1711.A.&B.&D.Based on observation at 3:15 p.m., S1 failed to meet the required child to staff ratio for children age 8-month-old to 1-year-old. The required ratio for children of this age is 5 children per 1 staff person. The Specialist observed S2 with 10 children, age 8-months-old to 1-year-old. One additional staff was needed to meet ratio. This was corrected when 5 one-year-old children were placed into another classroom with S4. Corrective Action:

1713.C. Based on observation at 3:15 p.m., S4, assigned to 10 children, age 8-months-old to 1 year-old, failed to know the age and number of children in her assigned care. Corrective Action:

1901.A.1. Based on observation at 4:15 p.m., S1 failed to have a functional, readily available telephone capable of incoming and outgoing calls at all times at the center. S1 stated that she is using her personal cell phone for this purpose. Corrective Action:

1711.A.&D.&E.: Based on record review and interviews at 10:15 a.m., S1 failed to meet the required child to staff ratio for children on 11/17/2025. S1 stated on 11/17/2025, she left the center at 3:00 p.m. and returned at 3:45 p.m. There were 2 staff present at the center supervising 25 children between the ages of seven months to four-years-old. There should have been 3 staff present to ensure ratio was met. Corrective Action: Effective 12/17/2025, S1 stated she now has someone to pick up her daughter daily so she does not have to leave the center to ensure compliance with this regulation.

Based on record review, S1 failed to operate within the center's licensed capacity, as on 7/09/2025, there were 38 children signed into the center, and the center has been approved for an indoor capacity of 37 children. Corrective Action: Effective 7/23/2025, S1 stated she will not accept more than the approved amount of children to ensure compliance with this regulation.

Based on record review at 1:00 p.m., S1 failed to ensure the daily attendance log for children included the last name of the child on 7/09/2025. The last name of 14 of 38 children was not included on the daily attendance log. Corrective Action: Effective 7/23/2025, S1 stated she will require each parent to sign children's full names each day or create a pre-made list of children's name to ensure compliance with this regulation.

Based on record review and interview at 3:45 p.m., S1 failed to maintain a cumulative file for each child. S1 stated she did not have files for all the children and was in the process of updating all of her files. Corrective Action: Effective 7/23/2025, S1 stated she will complete the required paperwork to maintain a cumulative file for each child to ensure compliance with this regulation.

Based on record review and interview at 1:00 p.m., S1 failed to meet the required child to staff ratio for children two-years-old to eleven-years-olds on 7/09/2025. On 7/09/2025, there were 38 children present at the center. S2 signed out for lunch at 12:00 p.m. leaving S1 responsible for 33 children, as S3 was in the infant classroom with 5 children, aged 6 months - one-year-old. Corrective Action: Effective 7/23/2025, S1 stated she will schedule staff as needed to ensure there are enough staff within the building to remain in ration and ensure compliance with this regulation.

Based on observations, at 1:00 p.m., S2 failed to supervise resting children in connecting classrooms. S2 was seated on the classroom floor, with shelving that blocked her view as C1 and C2 napped in the connecting classroom. S1 stated they usually allow C1 (ten-years-old) and C2 (eight-years-old) to sleep by themselves in the classroom since they are older than the other children. S1 stated she did not know this was unacceptable. Corrective Action: Effective 7/23/2025, S1 stated she will review the center's supervision policy with all staff to ensure compliance with this regulation.

Based on observations/interview at 1:00 p.m., on 7/23/2025, S1 failed to post the current weekly menu listing specific food items served for each day of the week. S1 stated she was working on the menu on her home computer, and did not bring it to the center. Corrective Action: Effective 7/23/2025, S1 stated she will create a menu and have it posted prior to the beginning of the week to ensure compliance with this regulation.

C.: Based on record review / interview on 1/10/2025, at 2:30 p.m., S1 failed to provide documentation that 1 of 4 staff received additional orientation within thirty days of date of hire. S4 did not have the required training.

Based on record review/interview on 1/10/2025, at 2:30 p.m., S1 failed to provide documentation that 3 of 4 of the center staff, were provided opportunities for staff members to obtain a minimum of 12 clock hours of training annually in the topics found in 1719(A) and (B).
