      The personnel/ volunteer (ten hours or more per month) record did not include a CF-FSP 5337 Child Abuse and Neglect Reporting Requirements form signed annually. CCF Handbook, Section 7.4, C Record Keeping [SR] Facility did not have staff forms completed for current year they expired in September. Director had all staff that needed updated form to sign them during this inspection. The facility is now in compliance with the standard. In the Childcare facility handbook on page 61 states Form 5337, Child Abuse & Neglect Reporting Requirements, must be signed on or before hire date and annually thereafter by all child care personnel.       A ratio of [1] child care personnel for [11] children is required. A ratio of [1] child care personnel for [12] children was observed. s.402.305(4), F.S. General Requirements [SR] Upon this Counselor's arrival, this facility was out of ratio in two classrooms. The two-year-old classroom had 12 children and 1 teacher, and the four-year-old classroom had 21 children and 1 teacher. The Director was in the infant classroom and explained that she had another staff coming to maintain ratio. Another staff person arrived and took over the infant classroom, the Director moved a three-year- old from the two-year-old classroom and moved 10 three's and four's from the four-year-old classroom into her classroom. Ratio upon arrival: Infants - 4 children - 1 staff Twos/Threes - 12 children - 1 staff Threes/Fours - 21 children - 1 staff Final Ratio was as follows: Infants - 4 children - 1 staff Ones - 6 children - 1 staff Twos - 11 children - 1 staff Threes/Fours - 11 children - 1 staff Fours - 11 children - 1 staff
