C.: Based on record review at 2 p.m., S1 lacked documentation that 1 of 7 staff, S7, completed orientation requirements within seven and thirty days of the first day present at the center and prior to having sole responsibility for any children. S7 failed to complete the DCFS Mandated Reporter training and the LDE Modules 1-3 trainings. Corrective Action: Effective 10/14/2025, S1 stated she will have S7 complete the missing trainings ASAP, and going forward will ensure all new staff complete the orientation requirements within the allotted timeframes, to ensure compliance with this…
