S3 failed to have a current certification. A class has not been scheduled. Corrective Action: Effective 6/4/2026, S1 stated she will review staff files quarterly to verify all CPR certifications are current and be able to schedule renewal trainings timely, to ensure compliance with this regulation.

Based on interviews at 11 a.m., S1 failed to prohibit the use of tobacco in any form on the childcare premises as a staff stated they occasionally (unknown dates) used their vape in the center's restroom. Corrective Action: Effective 6/4/2026, S1 stated she will have a meeting with staff on 6/4/2026, and remind them that they are not to use any prohibited items at the center and vapes should remain in their vehicles to be used off-site during their breaks, to ensure compliance with this regulation.

Based on record review and observations on 1/7/2026, at 9 a.m., S6 (DOH: 12/1/2025) and S7 (DOH: 11/17/2025) failed to have current certification in CPR and pediatric first aid within 90 calendar days from their date of hire and prior to assuming sole responsibility for 13 three-to-four-year-olds. Corrective Action: Effective 1/21/2026, S2 stated going forward she will instruct new staff without a CPR/PFA certification to remind them if they are ever placed in a classroom alone with children that they cannot be alone, to ensure compliance with this regulation.
