Corrective Action: Effective 6/4/2026, S1 stated she will review staff files quarterly to verify all CPR certifications are current and be able to schedule renewal trainings timely, to ensure compliance with this regulation.

Corrective Action: Effective 6/4/2026, S1 stated she will have a meeting with staff on 6/4/2026, and remind them that they are not to use any prohibited items at the center and vapes should remain in their vehicles to be used off-site during their breaks, to ensure compliance with this regulation.

Based on record review and observations on 1/7/2026, at 9 a.m., S6 (DOH: 12/1/2025) and S7 (DOH: 11/17/2025) failed to have current certification in CPR and pediatric first aid within 90 calendar days from their date of hire and prior to assuming sole responsibility for 13 three-to-four-year-olds. Corrective Action: Effective 1/21/2026, S2 stated going forward she will instruct new staff without a CPR/PFA certification to remind them if they are ever placed in a classroom alone with children that they cannot be alone, to ensure compliance with this regulation.
