1713.A.B.C. Based on interviews on 2/13/2025, at 11:30 a.m., S31, S32, and S33 failed to ensure children were under supervision at all times. On 2/10/2025, at approximately 9:40 a.m., the staff took 14 two year olds to the playground. At about 10:25 a.m., they begin to clean up and gather the children to go inside and noticed C1 age - two-years old was missing. S32 went to look for C1 and found her on the porch. During this time, C1 was able to return to her classroom, change her clothes, and return to the play yard without any of the staff noticing she was gone. The time C1 was unsupervised…
