There are five staff who do not have evidence of a completed center orientation in their files. Please ensure that these five staff have completed a center orientation and that evidence is in their staff files.

There is one preschool teacher who only completed 3 hours of continued training for the 2023-2024 training cycle. Please have this teacher complete 9 more hours of continued training as soon as possible.

There were 3 infants napping on the floor with nursing pillows. The staff were advised that infants could not nap with pillows and should be in the crib with nothing but a tight fitting sheet.

A sample of records were reviewed. There are several children who are either missing an emergency form, the form is missing some information, or the form has not been updated annually. Please ensure that all children have an emergency form that is complete and updated.
