      1103. A-C.: Based on record review/interview on 06/12/2025 at 12:30 p.m., S1 failed to immediately notify the Department, DCFS, and the Parent of a Critical Incident. On 05/20/2025, S11 (DOH 10/07/2024) was observed by S1, S7, and S8 using a prohibited method of behavior management. S11 grabbed C1 (3-year-old) by the hand, roughly sat him in a chair, and pointed toward his head and chest, stating, "You don't ever put your hands on my son again." On 5/20/2025, around 10:30 a.m., S11 (DOH 10/07/2024) was observed by S1, S7, and S8 using a prohibited method of behavior management. The Specialist observed S10 (DOH 5/28/2025) failed to have eligible CCCBC documentation before being present at the center or performing services. Corrective Action: Effective 06/12/2025, S12 stated they will sign off every 30 minutes on all provisionally employed staff to ensure compliance with this regulation.

Corrective Action: Effective 06/12/2025, S12 stated they will move children out of the space to ensure compliance with this regulation.
