Based on record review at 1:05 p.m., S1 failed have an application/staff information form to include name, date of birth, home address and phone number, training, work experience, educational background, first day onsite working with children for S4 (DOH: 10/14/2024). This was not completed prior to the Specialist departure.

C.: Based on record review at 12:00 p.m., S1 failed to have documentation that S4(DOH: 10/14/2024) LDE Key Training Module 1 was completed within 7 days and the LDE Key Training Modules 2-3 were completed within 30 days of first day present at the center for. S4's first day present at the center was on 10/14/2024. This was not corrected prior to Specialist departure.
