Corrective Action: Effective 5/08/2026, S1 stated she will enroll S5 in a Department approved CPR and pediatric first aid class next week to ensure compliance with this regulation.

Corrective Action: Effective 3/23/2026, S4 stated she will notify the owner to remove all hazard materials to ensure compliance with this regulation.

The left side gate failed to be permanently attached and secured. Corrective Action: Effective 3/23/2026, S3 stated she will notify the owner to secure the side gate to ensure compliance with this regulation.
