Based on record review/observations at 11 a.m., S5 (DOH: 2/10/2026) failed to have a current certification in CPR and pediatric first aid prior to assuming sole responsibility for five 2-to-3-year-olds. Corrective Action: Effective 5/08/2026, S1 stated she will enroll S5 in a Department approved CPR and pediatric first aid class next week to ensure compliance with this regulation.

C.: Based on record review at 11 a.m., the provider lacked documentation for 1 of 5 staff. S3 failed to complete orientation requirements within seven and thirty days of the first day present at the center and prior to having sole responsibility for any children. S3 failed to complete and have documentation for verification of the LDE Key Training Modules 1-3 and the DCFS Mandated Reporter Training. Based on record review/observations at 11 a.m., S3 (DOH: 2/10/2026) failed to have current certification in pediatric first aid and CPR prior to assuming sole responsibility for eight 2-to-3-year-olds. Corrective Action: Effective 3/23/2026, S4 stated she will accompany S3 in the classroom to ensure compliance with this regulation.

Based on observation at 10:15 a.m., the outdoor area failed to be free of hazards. The Specialist observed the following: 2 areas with rotting exposed wood and roofing material on the covered awning of the play yard; rotten wood on the ramp leading out from the center onto the play yard; wood removed from a previous ramp stacked in a pile. Corrective Action: Effective 3/23/2026, S4 stated she will notify the owner to remove all hazard materials to ensure compliance with this regulation.

Based on observation at 10:45 a.m., the outdoor play space lacked an enclosure that protects children from traffic hazards, prevents children from leaving the premises without proper supervision, and prevents contact with animals or unauthorized persons. The left side gate failed to be permanently attached and secured. Corrective Action: Effective 3/23/2026, S3 stated she will notify the owner to secure the side gate to ensure compliance with this regulation.

Based on record review/interview at 12:45 p.m., S1 failed to have authorization for C1, age 2-years-old, prescription medication to include circumstances for administering as needed medication and any applicable special instructions, by the parents. Corrective Action: Effective 3/31/2025, S1 stated she will complete an as needed medication authorization for C1 to ensure compliance with this regulation.

Based on record review at 1:00 p.m., S1 failed to have a written plan of action as evidence by: C1, age 2-years-old requiring emergency medications, such as an EpiPen or Benadryl. Corrective Action: Effective 3/31/2025, S1 stated she will have a physician complete an emergency medication plan and update it as needed every six months to ensure compliance with this regulation.

Based on record review at 12:30 p.m., S1 failed to obtain written parental authorization for a non-vehicular excursion held in October 2024 for the children enrolled at center. Corrective Action: Effective 3/31/2025, S1 stated she will have parental authorization completed annually for non vehicular excursions to ensure compliance with this regulation.

Based on record review at 12:30 p.m., S1 failed to maintain a record of all non-vehicular excursion activities as evidence of an excursion was held in October 2024. Corrective Action: Effective 3/31/2025, S1 stated she will have maintain a record for non vehicular excursions to include the date, time, list of children, staff and type of activity to ensure compliance with this regulation.

C.: Based on record review at 10:35 a.m., S1 failed to have documentation that S4 (DOH: 12/16/2024) LDE Key Training Module 1 was completed within 7 days and the LDE Key Training Modules 2-3 were completed within 30 days of first day present at the center. S4's first day present at the center was 12/16/2024.
