This Statement of Deficiencies was generated as a result of the on-site State licensure inspection conducted at your facility on 12/12/2025. Please respond to each deficiency and attach documents as requested for the deficiency it pertains to. Sign and submit your Plan of Correction within 10 business days of receipt. The facility is licensed for 111 children as a Center. The census at the time of survey was 50 children. No children's files and no staff files were reviewed. Based on record review the facility failed to ensure that all staff were fingerprinted within 24 hours of hire and added to the facility’s NABS Roster at time of inspection. All staff new/transfers need to be added to facility NABS Roster within 24 hours of hire/transfer. Please add the staff below: Kathy H. Upload staff's consent and release form as proof of fingerprinting and NABS roster as evidence that she has been added into NABS. Answer the 4 POC questions. Based on observation signatures. Four teachers were present. and interview, two of the four classrooms Attached is the agenda. THe responsible had inaccurate sign in sheets. In room teachers were: Kelly, Brock, Campos and #103, only 9 children were signed in but 11 Bautista. children were present. In room #105, 11 children were signed in but there were only 10 children in the room at the time of the inspection.
