Based on Record Review at 10 a.m., S5 failed to maintain a daily attendance record for children which accurately reflect name of the person or entity to whom the child was released. The Specialist observed attendance log for 4/17/2026, 4/22/2026, 4/24/2026 and 5/1/2026, which did not include the first and last name of the person or entity to whom the child was released. Corrective Action: Effective 5/5/2026, S5 stated she will check attendance logs daily, prior to departure, for release signatures, to ensure compliance with this regulation.

Based on Record Review, a CCCBC-based determination of eligibility for Child Care purposes from the department was not obtained for the Contractor, prior to entering the center.
