The Specialist observed that 14 out of 15 staff had CPR training. S15 did not have infant and child CPR and Adult CPR training. Corrective Action: Effective 5/8/2025, S1 stated she will keep track of expired trainings through spreadsheet, to ensure compliance with this regulation.

Based on record review and interviews at 11:30 a.m. on 2/4/2025, On 1/30/2025 at approximately 7:30 a.m., S12 and S13 failed to physically walk through the bus and inspect all seat surfaces, under all seats and in all enclosed spaces to ensure no child is left on the vehicle as evidenced by: C1, three-years-old, was asleep on the bus upon arrival to the school and was left alone on the bus when S12 and S13 went into the gas station from 7:34 a.m. -7:41 a.m.

Based on record review and interviews at 11:30 a.m. on 2/4/2025, C1, three-years-old, was left unattended on the school bus 800 for 7 minutes from 7:34 a.m.-7:41 a.m. while S12(DOH 9/4/2024) and S13(DOH 8/19/2024) went into the gas station on 1/30/2025. C1 was discovered by S12 when they arrived to the bus yard at approximately 7:56 a.m. S12 and S13 were both terminated on 1/30/2025.

On 1/30/2025 at approximately 7:30 a.m., bus 800 arrived at the school and the children began to exit the bus. S9 and S13 compared the number of children that got off of the bus instead of a visual passenger check. C1, three-years-old, was still asleep on the bus as S12 and S13 took the bus to the gas station and then to the bus yard.
