Based on record review/interview at 11:00 a.m., S1 failed to provide documentation that all staff on the premises and accessible to the children have current certification in infant and child CPR and Adult CPR through training approved by the department. The Specialist observed that 14 out of 15 staff had CPR training. S15 did not have infant and child CPR and Adult CPR training. Corrective Action: Effective 5/8/2025, S1 stated she will keep track of expired trainings through spreadsheet, to ensure compliance with this regulation.

1723. C.: Pediatric First Aid: Based on record review/interview at 11:00 a.m., S1 failed to provide documentation that all staff on the premises and accessible to the children have current certification in Pediatric First Aid training approved by the department. The Specialist observed that 14 out of 15 staff had Pediatric First Aid training. S15 did not have the required Pediatric First Aid training. Corrective Action: Effective 5/8/2025, S1 stated she will keep track of expired trainings through spreadsheet, to ensure compliance with this regulation.

1727.A. Based on record review at 11:15 a.m., S1 failed to provide documentation that all staff completed the online child abuse and neglect Mandated Reporter Training provided by DCFS. The Specialist observed that 13 out of 15 staff had Mandated Reporter training. S13 (DOH: 08-22-2023) and S15(DOH: 09-25-2023) did not have Mandated Reporter training. Corrective Action: Effective 5/8/2025, S1 stated she will complete checklist of trainings with expiration dates attached, to ensure compliance with this regulation.

Based on record review and interviews at 11:30 a.m. on 2/4/2025, On 1/30/2025 at approximately 7:30 a.m., S12 and S13 failed to physically walk through the bus and inspect all seat surfaces, under all seats and in all enclosed spaces to ensure no child is left on the vehicle as evidenced by: C1, three-years-old, was asleep on the bus upon arrival to the school and was left alone on the bus when S12 and S13 went into the gas station from 7:34 a.m. -7:41 a.m.

Based on record review and interviews at 11:30 a.m. on 2/4/2025, C1, three-years-old, was left unattended on the school bus 800 for 7 minutes from 7:34 a.m.-7:41 a.m. while S12(DOH 9/4/2024) and S13(DOH 8/19/2024) went into the gas station on 1/30/2025. C1 was discovered by S12 when they arrived to the bus yard at approximately 7:56 a.m. S12 and S13 were both terminated on 1/30/2025.

2107.C. Based on record review and interviews at 11:30 a.m. on 2/4/2025, S12 and S13 failed to conduct a visual passenger check completion of each trip prior to staff members exiting the vehicle. On 1/30/2025 at approximately 7:30 a.m., bus 800 arrived at the school and the children began to exit the bus. S9 and S13 compared the number of children that got off of the bus instead of a visual passenger check. C1, three-years-old, was still asleep on the bus as S12 and S13 took the bus to the gas station and then to the bus yard.

B.3.: Based on record review and interviews at 11:30 a.m., S1 failed to notify the parent of the following critical incident: On 8/20/2024, at 11:54 a.m., the kindergarten students walked through the pre-k 3 playground to get to their playground. C1, three-years-old, walked through the gate with the kindergarten students and teachers to their playground. C1 was on the kindergarten playground for approximately 4 minutes with kindergarten teachers. C1 was never unsupervised, however, her teacher's, S6 and S7, did not know her whereabouts. S6 and S7 were on the playground supervising a total of 9 three-year-olds, when C1 exited the pre-k playground and went to the kindergarten playground. S2 and S3's class, 9 three-year-olds, and S5 and S9's class, 6 three-year-olds were also on the playground.
