      and LIC 624 Unusual Incident Report in a timely manner. LPA observed a sensor working on the front door. Staff are counting the children at all times. Staff #1 was terminated. Licensee will have a meeting with the staff and provide proof of meeting on care and supervision by 8/5/22. 8 9 10 11 12 13 14 This was evidenced by the Licensee / staff failed to supervised Child #1 as he was able to wandered out of the facility through the front door and down an alley on July 11, 2022.
