      Based on observation of the playground, the licensee did not comply with the section cited above in one out of one that the outdoor playground was not repaired after being cited on 01/10/2025 for the gopher holes. The playground grass to be overgrown, limiting visibility of the ground surface and potebtial harzards such as gopher holes, which poses/posed a potential health, safety or personal rights risk to persons in care.       Based on the LPA's record review and discussion with the office admin. the licensee did not comply with the section cited above in one out of one, the Department was not notified within 10 days of the change in director. The previous director's last day was 11/24/2021. The new director was hired on 11/19/2021, which poses/posed a potential health, safety or personal rights risk to persons in care.       Based on the LPA, and staff observation, the licensee did not comply with the section cited above in two out of two. The playground has two uncovered plastic water drainage holes open and accessible to children. The water meter cover is rotting and coming apart, as well as exposed nails, which poses/posed a potential health, safety or personal rights risk to persons in care. POC Due Date: 12/20/2022
