1713.A.B.C. Based on observations at 12:13 p.m., S6 failed to ensure children were supervised at all times. The Specialists observed 7 children, age 4-years-old, unsupervised napping in classroom 1. Supervision was corrected at 12:16 p.m., when S6 walked back into the classroom. It is unknown how long the children were alone. Corrective Action: Effective10/28/2025, S1 states all staff that needs a break will contact the director or staff in charge before walking out of the classroom to ensure compliance with this regulation

The Specialist observed 14 children present in Classroom 13 that had a licensed capacity for 12 children.
