      Based on record review at 11:00 a.m., S1 failed to have documentation that the entire center and play yard was checked after the last child departed to ensure that no child was left unattended at the center for the following days: 2/28/2025, 3/10/2025, 3/11/2025, 3/12/2025, and 3/13/2025. Corrective Action: Effective 8/20/2025, S1 stated she will designate 2 staff to complete the visual check daily to ensure compliance with this regulation.

      Based on observation at 12:00 p.m., S1 failed to ensure medication sent to the center was not expired. The Specialists observed an EpiPen that expired on 3/31/2025. Corrective Action: Effective 8/20/2025, S1 stated she will review all medication sent to the center to ensure compliance with this regulation.

      Based on record review at 11:30 a.m., S1 failed to have documentation that S5, S8, S10, and S11 completed the annual online child abuse and neglect Mandated Reporter Training provided by DCFS. Corrective Action: Effective 8/20/2025, S1 stated she will set a reminder 5 days prior to expiration date of certificate to ensure compliance with this regulation.

      Based on record review/observation at 10:15 a.m., S1 failed to have documentation of a CCCBC-based determination of eligibility for child care purposes from the department obtained for S2 prior to the person being present at the center or performing services. The Specialists observed S2 was on premises 8/20/2025. S2 left the premises at 10:17 a.m. Corrective Action: Effective 8/20/2025, S1 stated she will not allow anyone on premises without an eligible CCCBC to ensure compliance with this regulation.
