Based on video footage review on 09/29/2025 at 10:30 a.m., although there is a behavioral management policy implemented, S5 (DOH 10/14/1985) used a prohibited method of discipline as children were subjected to physical punishment. On 9/12/2025 at 9:43 a.m., S5 lifted and yanked C2, three-years-old, by his right arm from a seated position, closer to her seat. On 09/12/2025 at 10:39 a.m., C3, three-years-old, was waiting in the classroom to wash her hands when S5 grabbed C3 by both arms, yelling in her face, and maneuvered her to the mat for large group time. 1911.J. Based on video footage review on 09/29/2025 at 9:43 a.m., S1 failed to ensure S5(DOH 10/14/1985) practiced proper lifting of a child. The Specialists observed S5 lift and aggressively yank C2, age three-years-old, by his right arm, from a seated position, closer to her seat during transition reading time in head start classroom 1. Corrective Action: Effective 09/29/2025, S1 stated she will remind staff of proper lifting of children practices to ensure compliance with this regulation.
