Based on record review and interviews at 9:11 a.m., S1 failed to notify the parent immediately and the Department within 24 hours of the following incident: On 09/23/2025, at an unknown time, a DCFS Case Worker came to the center to investigate the allegations of abuse involving C1, age 4-years-old, on 08/20/2025. Corrective Action: Effective 10/01/2025, S1 stated she will notify DCFS Child Welfare and the Department with all Critical Incidents that occur to ensure compliance with the regulation.

Based on record review and interviews at 9:11 a.m., S1 failed to maintain documentation of a daily attendance record for Visitor's to include first and last name, date of visit, arrival and departure times, and purpose of the visit. A DCFS case worker visited the center on 09/12/2025, at an unknown time. The DCFS Case worker did not sign in or out on the visitorâs log. Corrective Action: Effective 10/01/2025, S1 stated she will check the visitors daily attendance to ensure visitors are signing in and out to ensure compliance with this regulation.
