Based on record review at 2:30 p.m., S1 failed to have documentation 1 of 35 staff on the premises and accessible to children have current certification in infant, child, and adult CPR through training approved by the Department. S45 failed to have the current certification. A class has been scheduled for 3/13/26. Corrective Action: Effective 2/23/2026, S1 stated she will set a reminder to check with central office after the first 30 days to ensure certification is acquired to ensure compliance with this regulation.

1713.B. Based on record review and interview at 10:00 a.m., S1 failed to ensure children were under supervision at all times. On 12/18/2025, at 8:48 a.m., S34 was supervising 13, 3-year-old children, when C1 left out of Classroom #13 through the connected bathroom and entered into Classroom #12. C1 was supervised briefly by S30 who was supervising 17, 3-year to 4-year old children , before exiting out Classroom #12 front door at approximately 8:50 a.m. and walking to the front office. C1 was found by S39 inside her office playing with toys alone at approximately 8:55 a.m. C1 was unsupervised…

1711.A.B.D.E. Based on observations and interviews at 9:40 a.m., the Specialist observed S3 caring for 16 children, ages 4 - years to 5- year old in classroom # 1. The required ratio for four to five year old children is 15 children per 1 staff. One additional staff was needed to satisfy ratio. Ratio for the children was corrected at 9:50 a.m. when C2, age 4 years old was moved to classroom #2. Corrective Action: Effective 01/07/2026, S1 stated she will do random walk-throughs throughout the day to ensure compliance with this regulation.

1807.C. Based on record review at 9:50 a.m., S1 failed to have a CCCBC-based determination of eligibility for child care purposes from the department on 01/07/2026, for O2, and O3 prior to them being present at the center or performing services. Documentation did not include signature of the staff member stating accompanying O2 and O3 at all times while on the premises. Corrective Action: Effective 01/07/2026, S1 stated she will ensure all visitors have a CCCBC prior to entrance at the center to ensure compliance with this regulation.

1903.E.5 Based on observation at 10:00 a.m., S1 failed to ensure the outdoor play space had an enclosure with a permanent fence or other permanent barrier in a manner that protects children from traffic hazards, prevents children from leaving the premises with proper supervision, and prevents contact with animals or an unauthorized persons. The Specialist observed the gate open with a silver and black chain attached that was not properly securing the 2- year- old playground. Based on record review at 2:15 p.m., S1 failed to have documentation that S14 (First Day Present 8/11/2025) completed LDE Key Orientation Module 1 within seven days of the first day present at the center and prior to having sole responsibility for any children. S14 completed LDE Key Orientation Module 1 on 8/20/2025. Corrective Action: Effective 9/25/2025, S1 stated she will have new staff complete LDE Key Orientation Module 1 within the first 7 days of employment.
