On 10/16/2025, at 8:11 a.m., C1, 4-years-old, was subject to physical punishment when S3 plucked her on the forehead. This could not be corrected. S3 was terminated on 10/17/2025. Based on record review/interview at 9:45 a.m., S1 failed to ensure the center's daily attendance record accurately reflected the children on the child care premises at any given time. On 7/10/2025, C1, 5-years-old, was picked up by O1 at 12:00 p.m. C1 was not signed out until 3:43 p.m. This could not be corrected. Corrective Action: Effective 7/16/2025, S1 stated he will check Brightwheel twice daily to ensure all children are signed in and out to ensure compliance with this regulation.

Based on record review/interview on at 12:00 p.m., although the center has implemented their written behavior management policy prohibiting physical punishment, S9 (DOH 5/29/2024) failed to abide by the policy as she used a prohibited method of discipline on 7/20/2025. S16 stated she was informed by S1 that on 7/10/2025, S9, aggressively grabbed C1, 5-years-old, by the left shoulder. S9 was suspended for 3 days. This could not be corrected. Based on observations at 10:04 a.m., S1 failed to ensure children were supervised at all times. Specialist observed children leaving the classroom upstairs to go downstairs without S12 or S13. This was corrected prior to the Specialist departure. Corrective Action: Effective 7/15/2025, S1 stated he will talk to the staff about the supervision policy and letting them know the children must be supervised at all times to ensure compliance with this regulation.

1723.F. Based on record review at 10:30 a.m., S1 failed to ensure all staff completed training in pediatric first aid and CPR within 90 calendar days from their date of hire. S5 (DOH 5/30/2025) failed to have this certification. The Specialist observed S5 supervising 5, 2-year-old, alone. This was not corrected prior to the Specialist departure. Corrective Action: Effective 7/15/2025, S1 stated he will make sure that someone with CPR/PFA is in the classroom with S5 until she obtains her certification to ensure compliance with this regulation.
