Based on record review and interviews on 12/03/2025, at 1:00 p.m., S2 failed to notify the Department within 24 hours after being made aware of a critical incident involving C1, 3-years-old. On 10/30/2025, at approximately 4:30 p.m., the child's parent informed S2 that she found C1 alone and unsupervised in his classroom while picking him up from the center. 1711.A.&B.&D.&E.: Based on record review and interviews on 12/03/2025, at 1:00 p.m., S4, S5, and S6 failed to ensure minimum child-to-staff ratios were met on 10/30/2025. -At 3:49 p.m., before exiting the center, S4 merged her class of 7 children age 3-years-old with S5's class of 10 children age 4-years-old, leaving S5 supervising a total of 17 children age 3 to 4-years-old by herself. The minimum child-to-staff ratio for a mixed group of children 3 and 4-year-olds is 14:1. -At 4:30 p.m., S5 and S6 were supervising their classes on the center playground. 1713.A.&B.&C.: Based on interviews on 12/03/2025, at 1:00 p.m., S5 and S6 failed to ensure children were supervised at all times and not left alone in any room, as C1, 3-years-old, was found by a parent unsupervised and alone in his classroom on 10/30/2025, at 4:30 p.m., while the rest of his class was on the playground. It is unknown how long C1 was in the classroom alone. This could not be corrected prior to the Specialist's departure. S1, who was present at the center and not signed in, stated she does not record her attendance when she is present due to being salary, but knows she should per the regulations. This was not corrected. Corrective Action: Effective 12/1/2025, S1 stated she will start tracking her attendance when she is present at the center to ensure compliance with this regulation.

1707.A.1.&2.: Based on observations, record review, and interview at 10:45 a.m., S1 failed to be on-site at the center full-time during the day time hours of operation. S1 stated she is currently having to work between two centers until S2 completed the new director training and returns from leave. No attendance records were available to show when she was last present full-time at the center. This was not corrected. C.: Based on record review and interview at 10:45 a.m., S6 (first day working 10/27/2025) lacked documentation of having completed the DCFS Online Mandated Reporter Training. This training should have been completed by 11/4/2025. This could not be corrected. Corrective Action: Effective 12/1/2025, S1 stated she will have all new staff complete their new hire trainings once they have been hired to ensure compliance with this regulation.

Based on observations, record review, and interview at 10:45 a.m., S1 failed to ensure S6, who was observed providing direct care to 4 1-year-olds, was being supervised until documentation of training was available. S1 stated S6 attended the CPR/PFA training on 11/18/2025, but is awaiting her card. This could not be corrected. Based on observations at 10:45 a.m., S1 failed to ensure crawlspaces were inaccessible to children. Specialist observed one crawlspace on the side building 5, facing building 6 and accessible to children. This was corrected prior to the Specialist's departure. Corrective Action: Effective 12/1/2025, S5 stated that the center will get someone to cover all of the exposed crawlspaces and have staff check for hazards before going onto the playground to ensure compliance with this regulation.
