Based on record/Interview on 10/23/2025, at 2:00 p.m., S1 failed to have the required CCCBC-based determination of eligibility for child care purposes from the department for O1. S1 stated that O1 lived in the home for 3 months, starting in July 2025. S1 failed to obtain an eligible CCCBC for O1. Corrective Action: Effective 10/27/2025, S1 stated she will make sure all adults living in the home, whether temporary or permanent, will get an CCCBC to ensure compliance with this regulation.
