1713.: Based on observation at 11:45 a.m., S3 failed to devote her time to supervision of the children, meeting the needs of the children, and participating with the children in their activities. S3 staff was observed cooking lunch for the children and handing out plates instead of devoting her time to supervising the children. Corrective Action: Effective 5/5/2026, S2 stated there are normally 3 staff present. When there is additional staff present, the extra staff usually floats. S3 stated she will make sure there is an additional staff present to ensure compliance with this regulation.

Based on record review at 10:00 a.m., S1 failed to provide opportunities for continuing education to staff members who are left alone with children, or who have supervisory or disciplinary authority over children. S2 and S3 failed to have 12 continuing education hours from 9/1/2024 - 8/31/2025. This was not corrected prior to the Specialist's departure. Corrective Action: Effective 1/22/2026, S1 stated she will make sure staff gets the required 12 continuing education hours for the upcoming annual to ensure compliance with this regulation.
