      Based on observation, interview and record review, the licensee did not comply with the section cited above. According to the director, she is unable to find the disaster drill log, which poses/posed a potential health, safety or personal rights risk to persons in care. POC Due Date: 07/14/2023

      Based on interview and record review, the licensee did not comply with the section cited above. LPA asked the direcotr if the center already complete the water testing. According the director, the center has water leak in the kitchen and it require a repair before they can do the lead water testing, which poses/posed a potential health, safety or personal rights risk to persons in care.
