S2 and S3 failed to have the current certification. Documentation of certification expired on 4/30/2025 and 10/31/2025. Corrective Action: Effective 12/17/2025, S1 stated she will have staff complete training with an approved trainer 90 days before expiration to ensure compliance with this regulation.
