Supervision of children in the [One's Classroom] group was inadequate in that [one child was left on the playground when classroom returned inside building]. CCF Handbook, Section 2.4 General Requirements [SR] Licensing Specialist observed the one-year-old classroom gathered in the hallway with two teachers. Licensing Specialist observed the one-year-old classroom returning inside the building from the playground. Licensing Specialist observed the VPK teacher open the door from the playground and bring a one-year- old inside the building. Licensing Specialist observed that (both) one-year-old teachers were not aware that the child was left on the playground with the other classrooms. Technical assistance provided from the CCF Handbook: 2.4 Supervision 2.4.1 General Supervision Requirements A. Child care personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised. B. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location. C. Supervision standards apply at all times away from the child care facility, including during field trips, outdoor play, and when picking up or dropping off children at designated locations, such as bus stops, school, or a childs home. Children in the outdoor play area were not adequately supervised in that [a child was allowed to release the latch to the playground gate and run out]. CCF Handbook, Section 2.4.1, A General Requirements [SR] Provider self-reported that a child exited the playground by releasing the latch to the gate that led outside to the parking lot. Provider released camera footage of the incident to the Licensing Specialist. Licensing Specialist observed two teachers on the playground at time of the incident. Licensing Specialist observed the two teachers engaging in conversation and not positioned adequately to watch all the children in the playground. Licensing Specialist observed the children yelling to the student to stop playing with the gate latch. Licensing Specialist observed the child releasing the latch and running out of the gate to another fence area. Licensing Specialist observed one teacher following the child out of the gate and catching him before he reached the alarm gate that exited to the parking lot. CCF Handbook, Section 3.9.6, B Food and Nutrition [SR] Licensing Specialist observed jars of food and snacks in the infant room not labeled with childs first and last name. Technical assistance provided from the CCF Handbook: 3.9.6 Breastmilk, Infant Formula, and Food B. The provider must make sure all formula and food brought from home are labeled with the childs first and last name. The provider is responsible for the label; therefore, if the label is not completed by the parent, child care personnel must put the label on when the formula or food is received Corrective Action: Director provided tape to teacher and had children names added to food and snacks.
