Open packages of perishable or leftover foods were not [ labeled with date]. CCF Handbook, Section 3.9.2, D Food and Nutrition [SR] Specialist observed an open package of hamburgers that were not labeled. Director labeled the package and said they were opened earlier in the week. The Child Care Facility Handbook states on page 30: D. Opened packages of perishable or leftover food items must be properly covered or sealed in containers or bags, labeled with the date, and properly stored and discarded within seven calendar days. Diaper changing occurred on a surface that was not impermeable. CCF Handbook, Section 3.10.2, E Sanitation and Equipment [SR] Specialist observed on the 3 diaper changing tables that the padded surfaces were torn and needed replacing. The director called the owner and the owner brought in and installed the new padded surfaces. The Child Care Facility Handbook states on page 37: E. When children in diapers are in care, there must be a diaper changing area with an impermeable surface that is cleaned and sanitized or disinfected after each use. A napping or sleeping infant who is not capable of rolling over was not positioned on his/her back and on a firm surface, or was swaddled, and the provider did not have written authorization for an alternate sleep position from a physician in the childs record. CCF Handbook, Section 3.6.4, B (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Specialist observed an infant in a bouncy seat. Once informed, the staff member removed the infant and placed it in a crib. The Child Care Facility Handbook states on page 26: B. When napping or sleeping, young infants who are not able to roll over must be positioned on their backs and on a firm surface to reduce the risk of Sudden Infant Death Syndrome, unless an alternate position is authorized by a physician. Sleep sacks that fit according to manufacturers recommendations, do not restrict the infants arms, and will not slide up around the infants face may be used for the comfort of the sleeping infant; however, swaddling shall not be used unless authorized in writing by the childs physician. Written documentation of a physicians authorization must be on file at the facility. Documentation must include the childs name, childs date of birth, description of sleep position required, instructions for the use of any equipment needed, and length of time authorization is valid. The director responsible for the daily operation of the program did not have an active Director Credential. CCF Handbook, Section 4.7 The director of record ST credentials expired on June 3, 2025. A new director was put in place today during this inspection. AA has an active credential that will expire on September 18, 2027. The Child Care Facility Handbook states on pages 45-46: 4.7 Director Credential Section 402.305(2)(g), F.S., requires a child care facility to have a credentialed director. Every applicant for a license to operate a child care facility or a license for a change of ownership of a child care facility must document that the facility director has an active Director Credential prior to issuance of the license. An applicant for the Director Credential must meet the requirements referenced in CFFSP Form 5290, Florida Child Care and Education Program Director Credential and Renewal Application, which is incorporated by reference in 65C- 22.001(7)(k), F.A.C. CF FSP Form 5290 may be obtained from the Departments website at www.myflfamilies.com/childcare. All applications and documentation will be verified, and, if complete, the credential will be issued by the Department or its designated representative on CF-FSP Form 5252, Florida Child Care and Education Program Director Credential, which is incorporated by reference in 65C-22.001(7)(g), F.A.C. The facility's written plan of scheduled activities was not followed. CCF Handbook, Section 2.6 General Requirements [SR] Specialist asked the infant's staff if they were taking the children outside, the staff responded no. She then said they do take the infants outside in a stroller if the weather permits. There is a playground for the infants, however, they are not taken outside because there is no access from the classroom to the playground. Director and staff will follow the daily schedule and take the children outside weather permitting. Specialist made second visit that afternoon during their scheduled outdoor time and observed the children were outside playing. The Child Care Facility Handbook state on page 18: D. Include both active and quiet play. Active play includes outdoor activities a minimum of twice per day, weather permitting.

The hand washing sink was used for something other than hand washing in that [rinsing of bottles and pacifiers]. CCF Handbook, Section 3.10.2, C Sanitation and Equipment [SR] Specialist observed infant bottles and pacifiers on a drying rack on the counter near the handwashing sink. Director and infant staff removed the bottles, pacifiers and drying rack from the counter. The Child Care Facility Handbook states on page 37: C. Handwashing sinks must not be used for food service preparation, dishwashing, or food clean up.

Toys, equipment and/or furnishings available to the children in care were not age appropriate in that [in the one-year-old room toys that were 1 inch plastic blocks with magnets were observed.]. CCF Handbook, Section 3.11, A Sanitation and Equipment [SR] Specialist observed plastic 1-inch cubes with magnets in them. These could be a choking hazard for the children. The Director removed the cubes from the room. The Child Care Facility Handbook states on page 38: 3.11 Indoor Equipment Equipment, materials, furnishings, and play areas should be sturdy, safe, and in good repair. Provider should monitor the Consumer Product Safety Commission (CPSC) recommendations for use of equipment. Walls, ceilings, floors, furnishings, equipment, toys, and other surfaces should be suitable to the location and the users. They should be maintained in good repair, free from visible soil and clean. Equipment and furnishings must be arranged to allow direct supervision of children by child care personnel from all areas within a room/play area. Bookshelves, televisions, and other heavy furnishings must be stable and secured as to not pose a threat of falling over. In efforts to prevent children from climbing to dangerous heights or enabling them to reach hazardous items, chairs and other furnishings must be placed away from cabinets and shelves. The program should make accommodations to the program environment and schedule so that children with special needs may participate. A. A child care facility must make available toys, equipment, and furnishings suitable to each childs age and development and of a quantity for each to be involved in activities.

Outdoor play equipment suitable to each childs age and development was inadequate for the number of children in care. CCF Handbook, Section 3.12 A Sanitation and Equipment [SR] Specialist observed no outdoor play equipment for the children to play on or with. There are few outdoor toys for the children to play with. The owner is ordering new outdoor toys for the children to play with. The Child Care Facility Handbook states on page 38: 3.12 Outdoor Equipment A. A child care facility must provide and maintain enough usable equipment suitable to each childs age and development and of a quantity for each to be involved in activities.

Bottles and/or sippy cups brought from home were not labeled with childs first and last name. CCF Handbook, Section 3.9.5, B Food and Nutrition [SR] Specialist observed bottles not properly labeled with child's first and last name in the infant room. Provider labeled the bottles properly during this inspection. The Child Care Facility Handbook states on page 34: 3.9.5 Food Handling B. Bottles and sippy cups brought from home shall be individually labeled with the childs first and last name and shall be returned to the custodial parent or legal guardian daily.

Diapering occurred in a room that did not have a hand washing station or did not have a hand washing station in an adjoining area. CCF Handbook, Section 3.10.2, A Sanitation and Equipment [SR] Specialist did not observe a handwashing sink in the infant room. A sink was added, and specialist made an additional onsite visit on 10/3/2024 to monitor the sink. The Child Care Facility Handbook states on page 36: 3.10.2 Diapering Requirements A. When children in diapers are in care, a hand-washing station that includes a sink with running water, soap, trash receptacle, and disposable towels or hand drying machines that are properly installed and maintained shall be provided in the room or in an adjoining area which opens into the room.
