      The facility did not provide shade on the playground. CCF Handbook, Section 3.5, C (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] During the playground inspection, the licensing specialist noticed the usual structure that provided shade was gone. The director confirmed she can get another shade structure set up by 05.23.25. The Child Care Facility Handbook states: 3.5 Outdoor Play Area C. The outdoor play area must provide shade. Shade may be provided by trees, buildings, or structures.

      The personnel/ volunteer (ten hours or more per month) record did not include a CF-FSP 5337 Child Abuse and Neglect Reporting Requirements form signed annually. CCF Handbook, Section 7.4, C (Section 2.1 Health and Safety, Page 3) Record Keeping [SR] The licensing specialist reviewed all 11 staff files. Employee SS had an expired Child Abuse and Neglect form, the director had SS sign one while the licensing specialist was onsite. The Child Care Facility Handbook states: 7.4 Personnel Records Records must be maintained and kept current on all child care personnel, as defined by Section 402.302(3), F.S. These records shall be on-site, available for review by the licensing authority and must include: C. CF-FSP Form 5337, Child Abuse & Neglect Reporting Requirements, which is incorporated by reference in 65C-22.001(7)(l), F.A.C., must be signed on or before hire date and annually thereafter by all child care personnel.

      The director responsible for the daily operation of the program did not have an active Director Credential. CCF Handbook, Section 4.7 Director who was supposed to renew director credentials by 2/9/2024; was terminated on 2/7/2024. Counselor received notice via email. No onsite inspection conducted. Provider must have a new director by 8/7/2024. 4.7 Director Credential Section 402.305(2)(g), F.S., requires a child care facility to have a credentialed director. Every applicant for a license to operate a child care facility or a license for a change of ownership of a child care facility must document that the facility director has an active Director Credential prior to issuance of the license. An applicant for the Director Credential must meet the requirements referenced in CFFSP Form 5290, Florida Child Care and Education Program Director Credential and Renewal Application, which is incorporated by reference in 65C- 22.001(7)(k), F.A.C. CCF Handbook, Section 7.4.1,C (Section 19.5, number 1) Record Keeping [SR] In reviewing staff files counselor found that one staff did not have level 2 clearing house documentation on file. Staff provided at time of inspection. 7.4.1 Background Screening Documents Background screening documentation must be maintained for all child care personnel as defined by Section 402.302(3), F.S., which includes household members if the facility is located in or adjacent to the home of the operator. Background screening documentation must be on-site and available for the licensing authority to review. C. A copy of the eligible results, for the Level 2 screening, generated from the Clearinghouse must be on record for each personnel. If awaiting Out-of-State criminal history results, a copy of the DCF email informing of the individuals eligibility for a provisional hire status must be in the personnel file.
