On 3/23/2026, at 2:48 p.m., C1 was left in the classroom unsupervised for approximately 2 minutes while the staff and other children were en route to the playground. Based on record review and interviews on 01/14/2026 at 12:50 p.m., S1 failed to notify the Department and DCFS within 24 hours of the following critical incident: On 12/02/2025, 12/03/2025, or 12/04/2025, during drop off (time unknown), S2 was informed that staff allegedly stated to C1, three-years-old, that if he did not go to sleep, the police would be called on him.
