1901.J.&K.: Based on observations at approximately 11:00 a.m., S1 failed to ensure items that can be harmful to children, such as cleaning supplies and plastic bags were kept in a locked cabinet or other secured place that ensured they were inaccessible to children. In the 3-4 year-old classroom, Specialist observed 1 black spray bottle of cleaning solution, 2 white spray bottles of cleaning solution, 1 can of Lysol disinfectant spray and 1 roll of black trash bags underneath the classroom sink in an unlocked cabinet in reaching distance of the children. Based on record review, S16 failed to lift a child using proper lifting techniques. On 11/07/2025, at approximately 2:50 p.m., S16 lifted C1, 3-years-old, by one arm while assisting him off of the merry-go-round while on the playground. Corrective Action: Effective 11/21/2025, S1 stated she will retrain all staff regarding proper lifting techniques. S2, S10 and S11 will monitor the teachers on a daily basis to make sure they are following the proper lifting techniques to ensure compliance with this regulation.

1915.B.&C.:Based on record review and interviews at 12:00 p.m., S16 failed to document and immediately report to the parent an incident that occurred. On 11/07/2025 at approximately 2:50 p.m. while on the playground, C1, 3-years-old, was being pushed on a merry-go-round by S16. When the merry-go-round stopped, C1 got off and was dizzy which caused him to fall over and hit his head on a seesaw located nearby. Based on record review/interview on 07/14/2025, at 01:45 p.m., S1 failed to obtain a CCCBC-based determination of eligibility for child care purposes from the Department for S17 prior to the staff working on the premises. S17 was hired and began orientation at the center on 07/11/2025. S1 was not able to provide an eligible CCCBC prior to specialist departure. S17 vacated the premises at 02:05 p.m. Corrective Action: Effective 07/14/2025, S1 stated she will make sure she has an eligible CCCBC for all staff before working on the premises to ensure compliance with this regulation.

Based on record review and interviews on 6/23/2025 at 3:30 p.m., S8 failed to notify within 24 hours of the incident the Department of the following critical incident: On 05/22/2025 at approximately 4:45 p.m., O1 reported to S8 that C1, 3-years-old, was not fed her lunch provided by O1 because O1 discovered the lunch untouched in her lunchbox located in C1's backpack at pick-up time. Based on interviews at 10:00 a.m., although the center has a behavior management policy in place, S2 was observed/heard yelling at C1, 2-years-old, after he was left unsupervised on the playground by S13 and S14. Corrective Action: Effective 4/16/2025, S3 stated that staff will be trained to address staff members involved and not children when an incident occurs to ensure compliance with this regulation.

C.:Based on record review and interviews at 10:00 a.m., S2 failed to notify the Department within 24 hours and the parents immediately of the following critical incident: On 4/10/2025, C1, 2-years-old, was allegedly left unsupervised on the playground for about 3 minutes from 2:58 p.m. to 3:01 p.m. Corrective Action: Effective 4/16/2025, S3 stated she will submit all critical incidents to the appropriate agencies within 24 hours of being made aware to ensure compliance with this regulation.

Based on record review and interview at 10:00 a.m., S2 failed to ensure the center met the required child to staff ratio for children of the following age: 2 years-old. The required ratio for children of this age is 10 children per 1 staff person. On 4/10/2025, S13 was observed on the center's camera footage supervising 13 children on the playground, the youngest being 2 years-old. Corrective Action: Effective 4/21/2025, S1 stated staff will be paired together to ensure compliance with this regulation.

Based on record review and interviews at 10:00 a.m., S13 and S14 failed to ensure all children in their care were supervised at all times while on the playground, as C1, 2-years-old, was left outside unsupervised for about 3 minutes, from 2:58 p.m. to 3:01 p.m. Corrective Action: Effective 4/16/2025, S3 stated all staff will be required to continue completing the face-to-name sheet and headcounts during all transitions in and outside of the center to ensure compliance with this regulation.

Based on record review and interview at 10:00 a.m., S2 failed to ensure 2 of 4 new staff, S4 and S7, had an application on file. Corrective Action: Effective 4/16/2025, S3 stated all applications will be placed in each new staff members file after their interview to ensure compliance with this regulation.

C.: Based on record review and interview at 10:00 a.m., S2 failed to ensure new staff completed center specific orientation, LDE Key Training Modules, and DCFS Online Mandated Reporter training within 7 days of their first day present and prior to having sole responsibility for children.
