1903C Based on observations at approximately 1220 pm on 10112023 the indoor area was not free of hazards as Specialist observed a leaf blower and over stacked supplies in the walkway leading the classrooms Specialist observed on the play yard two ladders laying on the ground roundup weed killer and medal yard steaks S1 removed all hazards prior the Specialist leaving the center

1907C2 Based on observations at approximately 1225 pm cots or mats are not sanitized daily and each childs sleeping accommodations were not assigned to himher on a permanent basis as they were not labeled C1 C2 C3 and C4 were sleeping in cribs that failed to be labeled This could not be corrected during the inspection

2101A1 Based on observation at 1100 am S1 failed to ensure the center conformed to all state laws Specialist noted the registration tag for the license plate expired 72023 S1 stated she will be sending the vehicle to the repair shop today to make repairs and is not currently in use This was not corrected

2101A8 Based on observations at approximately 130 pm on 10112023 the center vehicle failed to be maintained in good repair as evidence by the door was broken off of the 15 passenger van S1 stated is was scheduled for repair and was not on the premises on 10122023 during the inspection

2103F Based interview at 415 pm on 10122023 The current passenger transportation log provided to the Specialist was incomplete as it did not include date the transportation is provided name of the child name of driver and staff members pick up and drop off locations time child was placed on the vehicle time child was released and name of the person or entity to whom child was released signature of staff person completing the log S8 stated that passenger log had not been completed for the any school days in the month of October 2023 The could not be corrected during the inspection
