      Based on interviews and record reviews at 11:30 a.m., there failed to be a full-time director or director designee on-site at the center during the daytime hours of operation. S1 stated S5, Director, last worked at the center on 12/20/2024 and has been on medical leave since this date. S1 stated she is in the process of attempting to qualify to become the qualified director designee; however, she has to complete additional trainings before she can submit all the required paperwork in to the Department.

S1 stated the center's director has been on medical leave since 12/20/2024. S1 stated she will be submitting the necessary paperwork to become the qualified director designee.

Based on record review at 12:00 p.m., S1 failed to have a master transportation log that shall include the names of the children, the pickup and drop off locations, and the authorized persons to whom the children may be released. During the inspection, S1 printed the needed forms and began to fill them out.
