Based on observations and record review at 11 a.m., S1 failed to ensure the center's daily attendance records for children accurately reflect the amount of children on the center’s premises at all times. The Specialist observed 37 children present at the center and the daily attendance log only showed 32 children were signed in. Corrective Action: Effective 05/29/2026, S1 stated she will monitor the logs to ensure the daily attendance of all students present, is accurate and complete at all times through the day, to ensure compliance with this regulation.

Based on observations and interview at 10:04 a.m., S1 failed to prohibit electronic devices for children under 2 years old. The Specialists observed five 1-year-olds, watching television while playing on the floor. Corrective Action: Effective 05/29/2026, S1 stated she will ensure no electronic devices are being played for children under 2 years old, and will continue to monitor each classroom to ensure that this regulation is met at all times.

C: Based on record review at 10:30 a.m., S1 failed to ensure within seven calendar days of the first day present at the center, and prior to assuming sole responsibility for any children, each staff member complete the DCFS online Mandated Reporter Training. The Specialist observed 1 of 7 staff members did not complete the DCFS online Mandated Reporter Training. S6 (First day present: 05/11/2026) did not have the DCFS online Mandated Reporter Training. 1901.J.&K.: Based on observations 10:05 a.m., S1 failed to ensure items that can be harmful to children, such as medications, poisons, cleaning supplies and chemicals, and equipment, tools, knives and other potentially dangerous utensils, were kept in a locked cabinet or other secure place that ensures they are inaccessible to children.
