Based on record review and interview at 9:30 a.m., while supervising a group of children, S19 did not devote her time to the supervision of the children, meeting the needs of the children, and in participation with the children in their activities. On 10/01/2025, at 4:46 p.m., S19 was observed on the center's video footage, sitting on the classroom floor completing a report, while six one-year-old children were present in the classroom. C1 and C2 became entangled near the classroom door, and both children fell to the floor. S19 helped them up, but C1 continued to cry. S19 called S1…

Based on record review and interviews at 9:30 a.m., S19 failed to properly lift C1 and C2 (one-year-olds) using proper lifting techniques. The Specialist observed video footage from the infant classroom on 10/01/2025, at 4:46 p.m. C1 and C2 fell on the floor near the classroom door. S19 walked over to assist them, and helped each child up by lifting one of their arms. Corrective Action: Effective 10/15/2025, S1 stated she has reviewed the center's proper lifting techniques with S19 immediately following the incident to ensure compliance with this regulation.

: Based on record review/interview at 12:00 p.m., S1 failed to have documentation of tornado drills that were conducted at least once per month during the months of March, April, May, and June. There was no documentation that tornado drills had been conducted in March of 2024. Corrective Action: Effective 04/15/2025, S1 stated that she will conduct annual tornado drills during the months of March, April, May and June each year to ensure compliance with this regulation.
