Based on record review at 12:25 p.m., S7 failed to provide documentation that 5 of 5 staff, S3 (DOH 2/15/2023), S4 (DOH 5/21/2021), S5 (DOH 1/16/2020), S6 (DOH 4/5/2023), and S7 (DOH 8/9/2021), completed the annual online child abuse and neglect Mandated Reporter Training provided by DCFS.. Corrective Action: Effective 4/24/2025, S7 stated she will set a reminder 1 month prior to expiration for staff to renew their annual online mandated reporter training to ensure compliance with this regulation.
