      Based on observations and record review at 10:00 a.m., S4 failed to place C1, two-month-old, on his back for sleeping, and failed to have written authorization from a physician as required to sleep in a different sleeping position. C1 was immediately placed on his back prior to the Specialist's departure. Corrective Action: Effective 8/25/2025, S1 and S4 will ensure all infants are placed on their backs to sleep, and written authorization from a physician will be received before allowing infants to be placed in different sleeping positions.

Based on record review/interview(s) and observations at 9:30 a.m., Although the minimum child/staff ratio was met, S6 failed to meet the required child/staff ratio plus one additional adult for non-vehicular excursions. On 2/07/2025, at 9:30 a.m., S6 walked along the sidewalk in front of the center by herself, with 5 infant children, in the Bye-Bye Buggy. S1 stated she was unaware an additional staff member, not included in the ratio, is required during non-vehicular excursions.

      Based on record review/interview(s) and observations at 9:30 a.m., S1 failed to maintain a record of all non-vehicular excursion activities to include the type of activity, date, time, list of children, staff and other adults. On 2/07/2025, at 9:30 a.m., S6 conducted a buggy ride along the sidewalk in front of the center, with 5 infant children. S1 completed the Non-vehicular Excursion Log prior to the Specialists departure.
