(Section 2.1 Health and Safety, Page 3) General Requirements [SR] During onsite investigation the licensing specialist observed that on the day mentioned in the allegations the facility was out of compliance with ratio as well as there was staff member present starting at 8:13 am by herself with 12 children, there was a mixed group that included 6- 1 year old, 5- 2-year-old, and 1- 4-year-old. The second staff member did not come in until 9:40 am. All 12 children were signed in between 8:13 am and 9:15am. The facility was out of compliance on this day as the ratio was 1 to 6 with 1 year old children in the mix. 2.3 Ratios The staff-to-child ratio, as established in Section 402.305(4), F.S., is based on primary responsibility for the direct supervision of children and applies at all times while children are in care. 2.3.1 Mixed Age Groups In groups of mixed age ranges, where children one year of age but under two years of age are included, one child care personnel must be responsible for no more than six children of any age group, at all times.

Please have second staff member attest by signing that a sweep was conducted. 2.5.2 Transportation Log A. A log must be maintained for all children being transported in a vehicle or on foot away from and/or to the premises of the child care facility. The log must be retained on file at the facility for a minimum of 12 months and available for review by the licensing authority. The log must include: 1. Each childs name, 2. The date and time of departure, 3. Time of arrival at the destination, 4. The signature of the driver (or in the case of travelling on foot, the signature of the child care personnel), and 5. The signature of a second child care personnel or person(s) authorized by the provider to verify the transportation log and that all children have arrived safely and left the vehicle (if applicable).

CCF Handbook, Section 2.8, B (Section 2.1 Health and Safety, Page 3) General Requirements [SR] The facility is out of compliance with the discipline policy as the facility self-reported the incident of witnessing a staff member being aggressive and mishandling children in the classroom. The facility is to conduct an all-staff meeting discussing the written discipline policy. All copy of the discipline policy is to be provided to each staff member to read and sign and date acknowledging that they read and understand the policy. A copy of the sign policy is to be placed in each staff members file folder. A copy of the meeting notes showing all staff signatures and at least a copy of the discipline policy with a staff members signature showing acknowledgment of receiving and reading policy for corrective action to the licensing specialist. 2.8 Child Discipline A. The child care facility shall adopt a discipline policy consistent with Section 402.305(12), F.S., including standards that prohibit children from being subjected to discipline which is severe, humiliating, frightening, or associated with food, rest, or toileting. The teacher admitted to popping the child for not laying down. The facility is to create a safety plan that all staff will follow to prevent an incident from occurring in the future. The safety plan will need to discuss what protocol that will be taken if a teacher shall become overwhelmed and shall need assistance in the classroom. A copy of the safety plan is to be provided to the licensing specialist for corrective action. 2.8 Child Discipline A. Even though, an incident report had been written and signed by the parent, the licensure specialist recommends that an all-staff meeting be conducted discussing the importance of documenting any/all incident/accident reports. A copy of the meeting notes which should include all staff signature is to be provided to the licensing specialist for corrective action. 6.4 Accident/Incident Notification A. These records shall be on-site, available for review by the licensing authority and must include: A. A complete employment application with the required statement pursuant to Section 402.3055(1)(b), F.S. B. Documentation of position and date of employment. C. CF-FSP Form 5337, Child Abuse & Neglect Reporting Requirements, which is incorporated by reference in 65C-22.001(7)(l), F.A.C., must be signed on or before hire date and annually thereafter by all child care personnel.
