The Yellow Classroom was unable to present a completed sign-in sheet. The Red/Green Room had two separate sign-in sheets that were completed during the start of the day. One sign-in sheet Labeled Green had four children listed and the second sign-in sheet was labeled blue listing 8children. Both sign-in sheets tallied together only listed 12 of the 16 children present. The classroom sign-in sheet did not account for all the children and staff in the classroom. Furthermore, a cut and scrap spray were also observed in first aid kit with an expiration date of 8/2024.This spray will need to be discarded. Reminders: - Add Eligibility Memos to all staff files. - Child Files: Make sure that parents are completing all admission paperwork and not just signing off on documents. - Nelda Z. needs to complete a Wellness Course and 19 more hours of annual training. Please email copies of Wellness course and remaining annual training certificates to inspector.
