Based on record review and interviews on 04/21/2026, at 10:09 a.m., S1 failed to maintain documentation of a daily attendance record for visitors to include arrival, departure times and purpose of the visit. On the 04/17/2026, O4 visited the center and was not signed in. This could not be corrected. Corrective Action: Effective: 04/28/2026, S2 she will monitor daily when visitors sign in and out to ensure compliance with the regulation.

      Based on observations on 04/21/2026, at 10:48 a.m., S1 failed to follow the center's Electronic Devices Policy prohibiting the use of electronic device activities for children under age two, as the Specialist observed · an iPad in use in the one-year-old classroom, playing a cartoon, while 5 one-year-olds were present.
