A three-year-old child was observed unsupervised for five minutes in Room 5 when the child was left in the room with both doors to the unit closed. The surveyor notified the assistant program director to come get the child.

Of ten staff records, one staff member has not demonstrated a practical application component in front of an instructor certified by a nationally recognized certification program when pediatric first aid and pediatric CPR certifications were obtained on-line.

A staff member in Room 4 was observed yelling at two preschool children who were running outside of Room 4, "Get back in here. You know better than this," and they grabbed one of the children by the arm pit pushing the child back into the classroom.

Floors were not swept from the noon meal in Room 4 when the floor was scattered with food crumbs and stick spills were observed under a table.

Staff to child ratio was not maintained in Room 2, licensed for 10 children 12 months to 2 1/2 years old, when there was one staff with seven children between the ages of 1-2 years old.

Terms of the license was not maintained in Room 6, licensed for 17 children 2 1/2 to school age or school age, when there were 23 children 2 1/2 years of age including school age children.

1% boxed milk is stored on the floor in the pantry and not six inches above the floor.

Per staff statement, food from dented cans and cans without labels are used.

Terms of license was not maintained according to the license in Room 1, licensed for children 2 - 3 years of age, when there were children 1 year 11 months to 3 years 2 months present in the classroom.

The floors around the base of the toilets in the children's bathroom are not free from cracks or in good condition.

The kitchen is not clean at all times when there is an ice cream cup melting on the floor of the pantry and there is food debris around the base of the cabinets in the kitchen.

The written lesson plan in Room 6 was not age appropriate for school age when planning for them to do color sorting, trace numbers and letters.

Written lesson plans were not accessible when in Room 2 the plans were dated 6/2/25 and in Room 4 they were dated 3/31/25.

School age children in Room 6 do not have equipment, materials and toys in a sufficient quantity for a choice of three activities.

The license capacity on the license was exceeded in Room 3, licensed for 21 children 2 1/2 to School Age, when there were 25 children between 2 1/2 years to nine years old present at the time of the survey.

Cleaning supplies with warning label to keep out of reach of children was unlocked and accessible in Room 4.

The premises was not clean at all times in the bathroom with dried urine around the base of the toilets.

Current lesson plans are not accessible to parents, staff and licensing specialists in Room 2.

There was a bag of opened cookies in the kitchen and a bowl of ravioli was covered with foil in the pantry in the kitchen. This was corrected by staff at the time of visit.

The proper diaper procedure was not followed in Room 7, when the staff did not wash the infant's hands. (Corrected at the request of the surveyor).

Staff member used prohibited punishment when saying to a child "Do I need to call your daddy and tell him how you are acting right now" and "You know what? I'm calling your daddy right now. That is just gross. You are gross."

Cleaning supplies in Rooms 1 and 6, five cans of paint in Room 3 and multiple medications in the office are in unlocked storage and accessible to children.

A window in Room 1 is opened and it is not screened or guarded.

Medication was administered to four children and the staff who administered the medication did not record their name on the form.

Out of twelve children's files reviewed, one child does not have a health assessment on file.

The refrigerator in the infant room does not have a visible thermometer.

Toothbrushes are not available for each child's use and the children are not brushing their teeth daily after meals.

A six-month-old was not moved to the crib after falling asleep on the floor in Room 7.
