Supporting documentation provided from a text message the staff member who texted another staff member admitting to committing the act. The staff member is to conduct an all staff meeting discussing the importance of following the discipline policy. A copy of the meeting notes including all staff signature is to be provided to the specialist for corrective action. The facility is also to create a safety plan to prevent a future incident of this nature of occurring again. By January 1, 2022 all vehicles used by child care facilities to transport children must be equipped with a reliable alarm system approved by the Department which prompts the driver to inspect the vehicle for children before exiting the vehicle. 1. Approved alarm systems must meet the following criteria: a. The alarm system must be armed or activated automatically when the vehicles ignition is turned on. b. The alarm system must be designed and installed so that the vehicle horn, siren or other type of audio alarm will sound if the driver/staff member does not walk to the rear or, in the case of a passenger van, the side entry point of the vehicle, to manually shut off or deactivate the alarm. c. The time delay from the time the ignition is turned off after activation of the alarm system until the alarm sounds shall be no longer than one minute. d. The alarm must be audible from the distance of 500 feet from the vehicle. e. The alarm system must be installed so that the driver must walk to the back of the vehicle to reach the deactivation mechanism. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. The fence needs to be prepared to prevent the opening at the base of the fence. 3.5 Outdoor Play Area. The outdoor play area must have adequate fencing or walls a minimum of 4 feet in height. Fencing, including gates, must be continuous and must not have gaps or opening larger than 3 inches that would allow children to exit the outdoor play area. The base of the fence must remain at ground level and be free from erosion or build-up to prevent inside and outside access by children or animals. These areas must have at least two exits, with at least one being remote from the buildings. If an outdoor play area was approved for usage by the Department prior to October 25, 2017, no new exits are required to be added to meet this standard. CCF Handbook, Section 2.8, B General Requirements [SR] A staff member at the childcare facility failed to follow the discipline policy as a parent observed on camera a teacher being aggressive with her child. After the director observed video footage it shows that the teacher first threw the child's shoe onto their cot and then scooped the child up and "slammed" the child aggressively onto their cot. The director immediately fired the teacher after reviewing video footage. Even though the teacher was immediately terminated the licensing specialist recommends that the facility conduct an all-staff meeting discussing the assigned discipline policy. CCF Handbook, Section 2.5.2 General Requirements [SR] During onsite inspection the licensing specialist observed that the transportation log for Thursday, did not have a second signature indicating that a second person conducted the sweep and signed off on the log that it was completed. A copy of the transported log for Thursday and remaining of week will need to be provided to the specialist for corrective action. 2.5.2 Transportation Log A. A log must be maintained for all children being transported in a vehicle or on foot away from and/or to the premises of the child care facility. The log must be retained on file at the facility for a minimum of 12 months and available for review by the licensing authority. The log must include: 1. Each childs name, 2. The date and time of departure, 3. Time of arrival at the destination, 4. The signature of the driver (or in the case of travelling on foot, the signature of the child care personnel), and 5. The signature of a second child care personnel or person(s) authorized by the provider to verify the transportation log and that all children have arrived safely and left the vehicle (if applicable).

Bottles and/or sippy cups brought from home were not labeled with childs first and last name. CCF Handbook, Section 3.9.5, B Food and Nutrition [SR] During onsite inspection the licensing specialist observed that all bottles in the infant room did not have first and last name on each individual bottle. The item was corrected at the time of inspection as the staff member went back and assured that all bottles had first and last name on each bottle. 3.9.5 Food Handling. Bottles and sippy cups brought from home shall be individually labeled with the childs first and last name and shall be returned to the custodial parent or legal guardian daily.

The facility did not have documentation to show child care personnel had begun the introductory training within 90 days of employment in the child care industry. CCF Handbook, Section 4.1 Refer to the supplemental for the staff member who has been hired since 11-1-24, has not begun the required training in order to work in the childcare industry. Staff member will need to begin the required training and provide the licensing specialist with proof of beginning training. 4.1 Beginning Training Child care personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the child care industry and successfully complete Departments training within 12 months from the date training begins. Training taken prior to employment in the child care industry does not constitute begin training. The begin training timeframes begin at the time of employment in the child care industry. Training completion may not exceed 15 months from the date of employment.

Child care personnel failed to ensure arrival and departure times were complete and accurate at the time of arrival or departure. CCF Handbook, Section 7.5, B Record Keeping [SR] The licensing specialist observed that each classroom failed to assure that all children were signed in on the attendance log. The person in charge stated that the facility also uses an electronic devise for attendance as well. After review of the electronic device, the specialist observed that children had not been signed in on this as well. The item was corrected at the time of inspection when the staff members went back and signed the children in. 7.5 Daily Attendance Daily attendance of children must be taken and recorded accurately by the child care personnel, documenting the time when each child enters and departs the program. Attendance devices used for the purposes of tracking attendance may be used, but personnel must ensure the accuracy of the documented attendance. Each classroom must have an attendance sheet/class roster for the group of children occupying that space. Attendance sheet/class roster must accompany the child care personnel and the group of children throughout the day should they leave the classroom.

The facility did not have documentation to show child care personnel had begun the introductory training within 90 days of employment in the child care industry. CCF Handbook, Section 4.1 Refer to the supplemental for the staff member who did not begin the required training in order to work in the child care industry. 4.1 Beginning Training Child care personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the child care industry and successfully complete Departments training within 12 months from the date training begins. Training taken prior to employment in the child care industry does not constitute begin training. The begin training timeframes begin at the time of employment in the child care industry. Training completion may not exceed 15 months from the date of employment.

For child care personnel to satisfy this requirement the training must be accomplished through one of following methods: Department's Health Safety and Nutrition course, Safe Sleep course, or the Early Learning Floridas Safe Sleep Practices. Documentation of training must be maintained on the Department's training transcript in the child care personnel record.

Supervision of children in the [ 3 year old ] group was inadequate in that [The child was observed on video left in her classroom by her teacher.]. CCF Handbook, Section 2.4 General Requirements [SR] Video footage showed a 3-year-old child left in her classroom by her teacher. The teacher admitted to leaving the child in the classroom and realized that a child was missing when she did a face to name when she was on the playground. Teacher admitted that after realizing a child was missing, she saw the child standing at the back door of the classroom through the window. The counselor recommends that the facility create a safety plan on how they are going to prevent something of this nature occurring again. The facility is to conduct an all staff meeting and each staff member is to review the safety plan and sign the safety plan indicating that they understand the safety plan and will abide by what the safety plan stipulates to assure the safety of each child enrolled at the childcare facility. The meeting notes must also include the importance of writing any/all incident and accident report writing. A copy of the meeting notes showing signatures of all staff in attendance. 2.4 Supervision 2.4.1 General Supervision Requirements A.
