    • Medication CCF Handbook, Section 6.5 CCF Handbook, Section 6.5 (Form DEL-SR-6200A)40-04

      A medication, [Nystatin], brought to the center for administration, was not in its original container. CCF Handbook, Section 6.5, C (Section 2.1 Health and Safety, Page 3) Health Requirements [SR] Nystatin was received by teacher from parent with medication form filled out by not in the original container. This was resolved when provider threw the medicine away. Technical Assistance given from the handbook: Medication 6.5 C. Prescription and non-prescription medication brought to the child care facility by the custodial parent or legal guardian must be in the original container. Prescription medication must have a label stating the name and contact information of the physician and/or pharmacy, childs name, name of the medication, and medication directions. All prescription and non-prescription medication must be dispensed according to written directions on the prescription label or printed manufacturers label and maintained at the appropriate temperature. Corrective Action: Provider is to have a staff meeting and retrain all staff about medication and provider will submit an outline of the training along with all staff members signatures.       A child was given [yogurt] after the facility was notified of a food allergy by the custodial parent or legal guardian. CCF Handbook, Section 3.9.3, G Food and Nutrition [SR] A child at the center was given yogurt for a snack when on the enrollment form and doctor's note provided clearly stated that child is allergic to diary. Technical Assistance given from the handbook: G. If a special diet is required for a child by a physician, a copy of the physicians order, a copy of the diet, and a sample meal plan for the special diet must be maintained in the childs file and followed. If the custodial parent or legal guardian notifies the program of any known food allergies, written documentation must be maintained in the childs file for as long as the child is in care. Special food restrictions must be shared with childcare personnel posted in an easily seen location that is not readily visible by parents or nonchild care personnel and followed. Corrective Action: Provider will need to submit in writing a plan to make sure allergies are known certainly by administrators and by staff in each room including subs and how they plan to update all lists and how often they plan to update allergy lists. Provider will need to have a staff meeting and have all teachers sign and submit via email teachers' signatures and outline of the trainings for allergies. Due Date 9/30/2025

      Child care personnel did not comply with the facility's written disciplinary and expulsion policies. CCF Handbook, Section 2.8, B General Requirements [SR] The provider disenrolled a child from the facility without following the written expulsion policy. The child is no longer enrolled at the center. The provider has rewritten the discipline and expulsion policy and has had parents sign the new handbook. Technical Assistance given from Handbook: 2.8 Child Discipline A. The child care facility shall adopt a discipline policy consistent with Section 402.305(12), F.S., including standards that prohibit children from being subjected to discipline which is severe, humiliating, frightening, or associated with food, rest, or toileting. Spanking or any other form of physical punishment is prohibited. B. The child care facility operators, employees, and volunteers must comply with written disciplinary and expulsion policies.

      The facility did not have documentation to show child care personnel had begun the introductory training within 90 days of employment in the child care industry. CCF Handbook, Section 4.1 Employee listed in confidential section started employment 09-05-2023 and did not start the DCF training until 02-20-2024. Technical Assistance given from Handbook: 4.1 Beginning Training Child care personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the child care industry and successfully complete Departments training within 12 months from the date training begins. Training taken prior to employment in the child care industry does not constitute begin training. The begin training timeframes begin at the time of employment in the child care industry. Training completion may not exceed 15 months from the date of employment in the child care industry in any licensed Florida child care facility. This may be accomplished by classroom attendance in a Department-approved training course, acquiring an educational exemption from a Department-approved training course, beginning a Department-approved online child care training course, or by receiving results from a Department-approved competency examination. The child care program is responsible for obtaining training documentation from child care personnel.

      The personnel/ volunteer (ten hours or more per month) record did not include a CF-FSP 5337 Child Abuse and Neglect Reporting Requirements form signed annually. CCF Handbook, Section 7.4, C Record Keeping [SR] Counselor observed a new employee who was missing this form signed. Employment started on01/24/2023. Technical Assistance given from handbook: 7.4 Personnel Records Records must be maintained and kept current on all child care personnel, as defined by Section 402.302(3), F.S. These records shall be on-site, available for review by the licensing authority and must include: A. A complete employment application with the required statement pursuant to Section 402.3055(1)(b), F.S. B. Documentation of position and date of employment. C. CF-FSP Form 5337, Child Abuse & Neglect Reporting Requirements, which is incorporated by reference in 65C-22.001(7)(l), F.A.C., must be signed on or before hire date and annually thereafter by all child care personnel.
