to LPA by 06/28/2024 stating how the facility will maintain ratios at all times and provide proof of staff training to LPA. 8 9 10 11 12 13 14 Based on interview with Director and staff, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care. On the day of the incident (05/09/2024) staff, S2 left staff, S1 in the room alone with 19 children. 8 9 10 11 12 13 14 LIC9224 must be signed by all parents currently enrolled.
