C.: Based on record review/interviews on 8/12/2025, at 1:00 p.m., S1 failed to provide documentation that 1 of 5 staff, S4 (DOH 7/29/2025), received orientation within seven days of the first day present at the center and prior to having sole responsibility for any children. S4 was working in the classroom with 9 children, aged 3-4 years-old. Corrective Action: Effective 8/12/2025, S1 stated she will have S4 complete the required orientation trainings immediately to ensure compliance with this regulation.
