S1 reported the incident to DCFS on 10/10/2025. On 10/08/2025, at 5:57 p.m., O2 texted S13 to inform her C2, three-year-old, told her that her teacher pulled her arm. Based on record review/interview on 4/29/2025, S21 and S23 failed to supervise children at all times and know the whereabouts of children, as on 4/14/2025, at 10:49 a.m., C1, a three-year-old, was left unsupervised on the playground for approximately 1-2 minutes. S21 and S23 counted their children when they returned to the classroom, and realized they were missing one child.
